Vaccination Affidavits Notice – August 2006

The ACIVU respectfully requests all adoptions agencies to pass the following reminders to adoptive parents and adoption facilitators:

Many parents are forgetting to prepare their vaccination affidavits before they come to Guangzhou. While parents can easily visit the U.S. Consulate’s American Citizen Services section to complete this form, this could require an extra, avoidable trip.
