A New form for IR3 cases - I864W (Intending Immigrant's Affidavit of Support Exemption)

The ACIVU respectfully requests all adoption agencies to pass the following reminder to adoptive parents and adoption facilitators:

Effective immediately, Form I-864W established by the Department of Homeland Security (DHS) is required to be completed for all IR3 cases.  We will start checking the new form on Jan 18, 2007.  Please follow the instruction below to complete the attached new form:

Part 1. Information about t intending immigrant [No.1, No. 3 and No. 4, please put the adopted child's information;  No. 2, U.S. address; No. 5. U.S. phone number; No. 6 (N/A); No.7 (N/A)].

Part 2. Reason for exemption (please check the second reason)

Part 3. Concluding Provision (the petitioner write his/her name and then sign for the adopted child with the date).
